According to the Noridian Medicare website,
Medicare contractors conduct an open participation enrollment period, every year, to give eligible physicians, practitioners and suppliers an opportunity to enroll in Medicare as participating providers or terminate enrollment in the participation program.
When Medicare providers complete and sign the participation agreement (CMS-460 Form) they agree to accept assignment for all covered services that are provided to Medicare patients. CMS Manual 100-04 Chapter 1 section 30.3.12 outlines the annual open enrollment process.
Each year, a Medicare provider may change their status by completing the status form and returning it to Medicare during the Enrollment period. If no change of participation status is desired, no action is necessary.
The deadline for changing participating status is December 31, 2016
